Awellness action plan is a set of activities that an individualintends to do in order to enhance the health status or improve theprocess of recovering from a given illness. The design of aneffective wellness action plan requires the assessment of one’swellness and the determination of activities that are likely toimprove the wellness (Copeland, 2014 and Whisenand, 2011). This papercontains a custom designed wellness action plan that will help meimprove my health status.

This action plan is designed to help me record and monitor differentactions that I will require to improve my health. I will rewardmyself at different points in time depending on the points scored. Anall the time response will be equal to 5 points, most of the timeequal to 3 points, and some of the time equal to 1 point.
